Columbus Health Department urges families to get ready for school early


Muscogee County Schools begin on August 7, 2013. Children entering a day care center, private school, or public school for the first time in Muscogee County are required by Georgia law to present two specific forms for enrollment that can be completed at the Columbus Health Department.

The Health Department is urging parents to get their children ready for school early instead of waiting until the back to school registration rush. 

The first form that can be completed at the Health Department is Form 3231, which documents the child's immunizations. The second is Form 3300, which documents the child's vision, hearing and dental screenings.

Parents are reminded to bring their child's immunization records with them to the Health Department.

Children getting ready to start school also need to present a certified birth certificate.  If the child was born in Muscogee County, the birth certificate is available from the Columbus Health Department's Office of Vital Records, 2nd Floor, 2100 Comer Avenue, Monday through Friday between 8:00 a.m. and 4:30 p.m.  

The cost for a birth certificate is $25 for first copy and $5 for each additional copy received at the same time for the same child.  A Georgia driver's license or state identification card is required, and either a parent or grandparent, or legal guardian may pick up certificates.

On August 3, from 9 a.m. to 7 p.m., the Columbus Health Department will be partnering with Walmart on Buena Vista Road to offer Back-to-School Vaccinations. Visit for a price list of available vaccines and accepted payment methods for this event.

Georgia Law also requires children under the age of 8 to be transported in an approved car seat or booster seat. Safe Kids Columbus will also be present to offer free booster seats to the first 50 qualified participants at the Walmart Back to School vaccine event on August 3.  Child must be present, be at least 4-years-old, and weight at least 40 lbs.
