(RNN) – The Obama administration is facing more fallout for glitches in the Affordable Care Act's website, healthcare.gov. The website's rollout continues to face technological difficulties that leave users and potential customers frustrated.

Health and Human Services Secretary Kathleen Sebelius told CNN that President Barack Obama was never told about the problems before the website's release date of Oct. 1. Congressional hearings began to investigate the website's problems and who is at fault.

A 12-year-old Nevada middle schooler shot and killed a teacher and wounded two students before taking his own life. Authorities say the handgun he used belonged to his parents; police have not released a reason for the shooting.

A fire broke out at a Citgo refinery just southwest of Chicago. No injuries were reported.

In Danvers, MA, high school student Phillip Chism, 14, is accused of killing a teacher with a box cutter. The teacher, Colleen Ritzer, 24, was found in the woods behind Danvers High School where she taught and where Chism was a student. Investigators said Chism incriminated himself during police interviews and school security cameras capture him on the premises. He has been charged with murder and is being held without bail.

More than 200 homes have been destroyed by massive brush fires in southeastern Australia. The areas most affected are in the Blue Mountains west of Sydney.

Apple announced the latest edition of its popular device – the fifth generation iPad. The lighter, thinner device, dubbed iPad Air, will begin at $499 and will hit stores starting Nov. 1. The new iPad Mini will be released in November as well – just in time for the holidays.
