PHOENIX (CBS5) -

Authorities have released the name of a driver killed after his car crossed a Phoenix street's center line and collided head-on with two other vehicles.

Phoenix police said the man has been identified as 32-year-old Mario Reyes.

Tuesday's crash happened near Indian School Road near North 61st Drive about 8:15 a.m., according to police, and closed Indian School Road between 59th and 62nd avenues.

Reyes, the driver of a brown older model Buick LeSabre, crossed the center line and collided with a silver 2000 Hyundai and a Subaru Outback, Phoenix police Sgt. Trent Crump said.

Reyes was wearing his seatbelt and his airbag deployed, police said. 

Three people were taken to hospitals with life-threatening injuries, including Reyes, who later died.

The driver of the Subaru was wearing his seatbelt and his airbag deployed, officers said. The driver of the Hyundai was also wearing her seatbelt and her airbag deployed.  The front seat passenger of the Hyundai was not wearing her seatbelt and suffered injuries when she hit the windshield. 

A fourth person was treated and released at the scene, said a Phoenix Fire Department spokesman.

Crump said the road between 59th and 62nd avenues would be shut down through the evening rush hour.
